Summary The incumbent serves as a Senior Marriage and Family Therapist in the Integrated Behavioral Health Care (IBHC) team, a multi-disciplinary team within Mental Health Service. The team is co-located with Primary Care. Services include provision of high quality clinical mental health assessment and brief treatment, care management, crisis intervention and advocacy services, and the coordination of linkages to other appropriate VA or community service providers/agencies as needed by the veteran. Responsibilities This position is open to teleworkers and virtual employees in the same geographic area. Major duties and responsibilities include but are not limited to the following: Screens and accepts cases referred to IBHC and provides brief assessment and treatment to appropriate cases or refers cases to the best treatment site. Makes an independent assessment of the psychosocial, health care, and financial needs of each veteran, diagnosing mental and behavioral health conditions; refers veterans with severe and chronic symptoms to the most appropriate program to address their needs. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team. Implements evidence-based treatment modalities, providing psychotherapy for individuals, families and groups in the outpatient, primary care setting, and measures outcomes of interventions. Carries full professional responsibility for cases presenting with a wide range of psychosocial and environmental problems. Implements evidence-based treatment modalities for health-related concerns such as medication compliance, adherence to medical treatment recommendations, medication adherence, smoking cessation and harm reduction for alcohol or substance use. Works closely with medical professionals to coordinate these interventions. Documents all services provided in the veteran's VA Health Care System electronic medical record. Works closely with VA and community health care professionals to provide and support treatment plans for veterans. Provides consultation to health care personnel on the psychosocial components of the veteran's situation. Identify, develop, and facilitate accessibility and utilization of VA and community resources to meet the needs of IBHC veterans. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Collaborate with members of the IBHC team and other professional specialty clinics to provide comprehensive care to IBHC veterans. Identifies high risk patients and provides care management services until patients are engaged in a higher level of care. Work Schedule: Monday to Friday, 8:00 a.m. - 4:30 p.m. Telework: Available. Education. Candidates must meet one of the following:(1) Hold a master's degree or doctoral degree in marriage and family therapy from a program approved by COAMFTE,(2) Hold a master's degree or doctoral degree in marriage and family therapy from a MFT program from a regionally accredited institution, OR,(3) Hold a master's degree or doctoral degree in a comparable mental health degree (Licensed Professional Mental Health Counselor, Social Work, Psychiatric Nursing, Psychology, and Psychiatry) that meets the current VA qualification standard of that profession. Licensure. Persons hired or reassigned to MFT positions in the GS-0101 series in VHA must hold a full, current, and unrestricted MFT license to independently practice marriage and family therapy in a State. Physical Requirements. See VA Directive and Handbook 5019. English Language Proficiency. MFTs must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, paragraph 3j. Grade Determinations: GS-12 Education, Experience and Licensure. Candidates must have successfully completed evidence based training in family therapy and must have graduated from a COAMFTE accredited program OR have passed the AMFTRE. Candidates must have at least five years of post-licensed clinical experience in delivery of family systems therapy including one-year equivalent to the GS-11 grade level. This experience must demonstrate the use of an evidence based family therapy model while exhibiting progressive professional competency and expertise. Assignments requiring responsibility for providing clinical supervision must have an AAMFT Approved Supervisor credential. In addition, the candidate must demonstrate the professional KSAs listed below: 1. Ability to provide specialized consultation for complex cases involving individuals, families and/or couples utilizing evidenced-based family systems theory/therapy. 2. Ability to provide crisis intervention which includes assessment, safety planning and complex treatment planning using independent clinical judgment utilizing family systems theory/therapy. 3. Ability to provide family systems therapy, which seeks to address Veterans and their family members on an individual level and deals with interpersonal interactions. 4. Ability to provide unlicensed MFT staff/trainees with systemic clinical supervision towards licensure from a family systems theory/therapy perspective. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Advanced MFTs can be assigned as the sole licensed mental health provider in settings such as CBOCs or satellite outpatient clinics or have responsibility and oversight for a MFT training program established under VA/VHA training policy as well as provide systemic clinical supervision to those MFT interns. The Advanced MFT who has responsibility for the training program provides family systemic clinical supervision to unlicensed MFTs and provides clinical consultation to less experienced staff. Advanced MFTs develop behavioral health related training curriculum and conduct trainings for clinical staff. Advanced MFTs meet routinely with inter-professional team members and leadership to communicate about program and veteran and family issues for in depth problem solving. Physical Requirements: The work requires extreme patience and controlling of emotions. The work may also require assisting physically disabled veterans. Day-to-day work involves sitting, walking, standing and carrying light objects, such as medical records. Work is typically performed in a clinical setting that exposes the incumbent to the communicable disease common of the homeless population. Much of the work is performed in a setting away from the office, where the environment cannot be controlled.
